Great News! We have partnered with ARAG – our group legal plan provider – to help you create these essential healthcare legal documents for free:


  • Living Will – a document containing your wishes regarding the use of extraordinary life support or other life-sustaining medical treatment when your condition is medically without hope of recovery, or death is imminent.

  • Healthcare Power of Attorney – a document allowing you to grant someone permission to make important medical decisions on your behalf in the event you are unable to make them yourself.

  • HIPAA Authorization (Patient) – a form enabling certain trusted family and friends to have access to your medical records and health information when you are injured. These records are typically used to help prove whether you are capable to make healthcare and financial decisions on your own, and therefore could activate a Healthcare Power of Attorney or a Durable Power of Attorney that you signed when healthy.

Creating these documents can eliminate undue stress, even legal action, between loved ones who may be faced with difficult decisions regarding your care.


How it Works – Getting Started is Easy

  • Register and set-up your password. Your user name and password will be e-mailed to you for safekeeping.
  • Login by selecting “Existing Users: Login.”
  • Enter your user name and password. Click “Go.”
  • Choose the appropriate links to create your free healthcare legal documents.

Please note: Your access to this site and your newly created documents are for a limited time only and will expire at the end of your benefits open enrollment period – November 13, 2009. To prevent the loss of your online legal documents, we recommend you download and save these documents to your computer prior to the end of your enrollment period.
